Alerts stop repeating for resolved incidents

A resolved incident no longer sends the same alert again when a flapping monitor recovers.

Flapping monitors were the loudest bug report of the summer. A monitor that failed, recovered and failed again could page the same person ten times in an hour.

Fixes

Alerts for the same monitor are grouped while an incident is open.
A recovery within 5 minutes of a failure no longer starts a new incident.
Resolved incidents are not re-alerted when a monitor is edited.

In our own data this cut notifications per resolved incident by 38 percent.
